Why is the Attain™ IVF Refund program the best option?

What is the chance of having a baby through IVF treatment?

While there are no guarantees that you'll be able to have a baby through IVF, you will dramatically increase your chance of success when you commit to a course of treatment plan.

Who qualifies for the program?

Once IVF is recommended by one of our participating physicians as an option when faced with infertility, IntegraMed's clinical staff will review the following information to determine if you qualify for the Attain™ IVF Refund Program.

Criteria can include:

• Women's age - Ability to complete all three IVF cycles prior to the 38th birthday (unless donor eggs are being used)
• Day 3 hormone tests - FSH and E2 within normal limits
• Semen analysis
• Evaluation of fallopian tubes and uterine cavity
• Body Mass Index (BMI)
• Need for Preimplantation Genetic Diagnosis testing (PGD)
• Medical history, including highest recorded FSH, prior IUI and IVF cycles, and pregnancy loss history (miscarriage)

Acceptance into the Attain™ IVF Refund Program is at the sole and subjective discretion of IntegraMed. Please remember that IntegraMed is making a determination about your participation in a financial program, not a clinical determination of your chances of success with IVF. What if I don't qualify for the Refund Program?

What's the cost of IVF through the Attain™ IVF Refund Program?

Your fee will vary depending upon the practice's pricing, your age and your customized treatment plan. Your Attain IVF Customer Care Specialist will discuss IVF fee options with you once your application has been processed. There is no cost to apply or obligation to participate.

What if I get pregnant after the first cycle?

Congratulations! That's what patients who undergo IVF treatment are hoping for. In fact, in a recent survey of our patients, 98% who get pregnant after the first cycle would recommend Attain™ IVF to their friends. No one knows for sure if you will get pregnant right away, so in the event that you succeed in your first cycle and pay more than a "pay as you go" patient, eliminating the unknown financial concerns provides the peace of mind you need so you can focus on having a baby and not on the associated costs.

What if my insurance plan covers IVF?

Do you offer financing?

Yes. In addition to your own credit options, we offer access to IVF financing options to allow you to minimize your cash payment. Your Attain™ IVF Customer Care Specialist can assist you in understanding your fertility financing options, including using limited insurance benefits.

There are other IVF refund programs available, why is Attain better?

• Attain™ IVF is the largest refund program in the United States. It is administered by IntegraMed America, a publicly-traded company (Nasdaq: INMD), a leading provider of specialty healthcare services serving the fertility market since 1984.
